Dear Colleagues and Friends,
Covid 19 pandemia is a major, novel and lasting public health and life-threatening issue.
It is expected, from the Chinese experience, that Covid 19 would possibly be, in France, to an end by mid-may that would make realistic a June Dax ISMH Congress. Nevertheless, this is not sure and are also unkown the evolutions of the disease in the different countries, in Europe and worldwide.
At the present time the probability of a Congress in June appears to be very low.
We are working with all our team on other possibilities, mainly later in the year or eventually next year if it is impossible for this year.
We’ll discuss with the ISMH president and board of the final decision to take when we’ll have robust proposals at our hand.
We expect to announce the final decision on April 14th.
I engage all of you to delay hotel and flight booking but to continue submission of abstracts for the Congress via the website : submission.

My very best and friendly regards.

Professor C-F. ROQUES LATRILLE
President of the 45th ISMH CONGRESS Organizing Committee

Welcome by the President of the Congress

20 centuries ago, the troops of the famous roman general Cneius pompeius magnus, were already benefiting from the Dax maturated muds. The roman soldiers returned home long ago, but patients with musculo-skeletal conditions continue to be successfully treated. The peloids made Dax one of the most antique, much-acclaimed and active spa resort in France.

20 years ago, an academic institute was created in Dax. It is dedicated to the training of professionals for balneotherapy care facilities and other hydrotherapy activities as well as to scientific research. The Dax institute will host the venue of the 45th ISMH Congress. We hope the delegates will share the best and most novel of the scientific knowledge and professional experiences.

2020 june 18, 19, 20, Dax will be proud to offer you the traditional and celebrated hospitality of the south-west of France. I am sure all of you will enjoy it..

Welcome in Dax.

Professor Christian-F. Roques MD

President of the congress

Address by the President of the ISMH

Dear Colleagues and Friends,

Welcome to the 45th World Congress of Medical Hydrology.

The International Society of Medical Hydrology (ISMH) is proud to have its 45th Congress in Dax under the Presidency of Prof. Christian-François Roques-Latrille.

France has a great tradition in Balneology. Not only historical but also in scientific, social and cultural aspects. Dax is a very good example of this with its rich history and its Institute of Thermalism where Education and Research get together allowing a very important development on Balneology knowledge. Thus, having an ISMH Congress in this thermal city constitutes a very special occasion. We are expecting participants from all over the world. ISMH is spreading its presence on more places allowing the contact of scientists from different countries, regions and cultures. With this we are giving people more opportunities to share their knowledge and experience with their colleagues from all over the world. I am convinced that the presentations within the proposed topics will reach a high level and will interested you. Besides the scientific contents this congress aims to increase the participation of all the ISMH members in its activities. To start this process we would like to invite you all to our General Assembly to share and discuss our ideas and future plans. We will talk there about statutes review, website renewal, financial situation, next congresses, international courses, cooperation with other balneology institutions.  For this we do need the contribution of all of you.

In conclusion we may say that this 45th Congress has many important aspects to the future of Medical Hydrology. We trust on a good participation in the success of the organization. Wish you a fruitful meeting and a wonderful stay in Dax that can bring you excellent memories.

Sincerely yours,

Pedro Cantista,
ISMH President

1142 - Implications and possible interventions based on natural therapeutic factors in hypoxia on cerebral level – scientific data and synthetic integrative correlations of related previously achieved and specifically focused studies

Fri, Mar 6
6:00 PM – 7:00 PM

Objectives:

The central nervous system is intolerant to ischemia and 10-30 seconds without oxygen can initiate damages in neuronal, of support and vascular, structures, that can lead to irreversible effects (most serious: cells death) after more than 5 minutes of (quasi) total arrest of blood supply in a brain territory. Hypoxi/ischemic injuries in stroke are devastating conditions that can affect individuals of all ages, possibly occurring perinatally - with consequent encephalopathy - and respectively, in adults and elderly, being one of the major causes of death and disability. Our objectives target, first, to have a better understanding of the actual scientific description of the chronic ischemia detrimental outcomes on cerebral level and secondly, to present some of the intervention possibilities aiming to reduce the consequences of stroke. Despite considerable research, there are still no proven clinically effective pharmacological industrial agents capable to spectacularly mitigate the severity of brain lesions following stroke.

Design:

We achieved prospective longitudinal analyses in, totally, 55 chronic post-ischemic stroke patients, mean age 69 years, which made rehabilitation treatment: the first group, of 20 patients, performed only kinesitherapy, massage and occupational therapy, during 16 days, at the Clinical Rehabilitation Hospital in Cluj-Napoca, and the second group, of 35 patients, performed the same treatment during 16 days, plus in association with carbonated mineral water baths for 15 minutes daily, mofette therapy for 20 minutes two times per week, kinesitherapy and massotherapy 15 minutes daily, each,  and aerotherapy 30 minutes, daily, at Tușnad Spa Complex, in Băile Tușnad. Hemiparesis was the most frequent clinical sign, followed by coordination, balance and gait, disorders. Every patient, within both mentioned settings, was clinically assessed before and after treatment, based on: Motor Assessment Scale, TINETTI Balance Scale, 10-m walk test, BARTHEL Index, and Quality of Life Scale, in both groups. Safety issues/adverse reactions were noted.

Results: The mean scores, at baseline and after therapy, in each setting, on the scales used, were compared with the paired T-test. On the Motor Assessment Scale, a statistically significant improvement was determined (p < 0.05). When evaluating the patients’ performance, using the Barthel Index, and balance on the TINETTI scale, improvements were statistically significant, too (p < 0.05).  At the end of the treatment, we also observed an improvement in the walking speed (p< 0.05). Quality of life showed statistically significant improvements (p < 0,05), in both groups. So natural therapeutic agents: carbonated mineral waters and mofettes, together with aerotherapy, have shown, alternatively, good results. There were no adverse reactions to the treatments, neither in the Clinical Rehabilitation Hospital in Cluj Napoca nor in Tușnad Complex, Băile Tușnad. Yet, the stroke survivors evaluated remained with important limitations on participating in social activities, reintegration into the family, community and working life.

Conclusions:

Natural therapeutic factors, such as carbonated mineral water baths and mofettes, possibly along with aerotherapy, prooved useful for rehabilitation treatment in chronic post-ischemic stroke patients, overall influencing the clinical and functional picture and determining significant improvement of their: motor status, autonomy, balance, gait speed, and quality of life. Of the more than 100 ”neuroprotective” agents studied in randomized clinical trials on focal ischemic stroke, none has proven unequivocally efficacious. However, the failed trials have greatly increased our understanding of the fundamental biology of ischemic brain injury and have laid a strong foundation for future advance. New intimate pathophysiological targets continue to be identified, which is a fertile area for translational medicine, including in stroke. To avail rehabilitation treatment complementarily in balneal medical spa resorts, using natural therapeutic agents, as emphasized in our previous studies, in the absence of contraindications, is very important, necessary and useful for chronic post-ischemic stroke patients.
